Need a D.O.T. SAP Assessment under
U.S. D.O.T. 49 CFR Part 40?
Select a pro: Margie Roop, LPCC-S; CEAP; SAP has conducted DOT SAP Assessments since 2007. The Initial SAP Evaluation takes two-hours where Margie will conduct a thorough assessment and make recommendations. Once the recommended treatment has been completed, the one-hour Follow-Up SAP Evaluation will take place. Margie will conclude if the recommendations were successfully completed, make aftercare recommendations, if appropriate, and develop your Follow-Up Testing Plan. The required written Initial SAP Evaluation and Follow-Up Evaluations will be sent to the Designated Employer Representative (DER). In most cases, an Aftercare Plan will be developed and monitored by the DER.

You Must Agree to the Following:
I agree to pay a total of $620 credit card/$600 cash, money order, cashier's check, Venmo, Cash App for SAP services at the time of the Initial SAP Evaluation; if not paid in full, the Evaluation will be cancelled and rescheduled with a fee of $50 added to the total fee and due at the next appointment.
I understand that other than an emergency, should I need to cancel my Initial SAP Evaluation, I must do so 24 hours prior to the appointment. If I do not, then a $50 cancellation fee will be added to the total and due at the next appointment.

I understand that if I do not show or call to cancel my Initial SAP Evaluation, Margie Roop will not re-schedule the Evaluation.

I understand that when I arrive for my Initial SAP Evaluation, I may pay via credit card, cash, money order, certified cashier's check, Cash App,, or Venmo.

I understand that if my job falls under the US DOT Federal Motor Services Carrier Administration (FMSCA), that Margie Roop will not approve my SAP request on the FMSCA Clearinghouse until the above fee is paid.
